Referring Your Patient

TAPMI’s central triage and collaborative care model

In the TAPMI model, referring practitioners play an active role in the treatment of their patients.

The TAPMI team will provide assessment and a care plan for your patients' chronic pain problem. In some cases, treatment may be initiated by TAPMI, however, once stabilized (6– 24 months) the patient will be returned to you for ongoing care, including pharmacotherapy, with our continued support.

TAPMI physicians and nurse practitioners will not take over prescribing opioids.

New Referrals

Consider a referral to TAPMI when:

Patient has persistent pain beyond 3 months that is not responding to current treatment, and is showing evidence of:

  • Declining physical functioning
  • Absence from work or multiple emergency room or walk in clinic visits for pain management
  • You would like guidance on patient opioid use, withdrawal or weaning.
  • Suspected Complex Regional Pain Syndrome (refer early- < 6 m from the start of symptoms)
  • Well controlled medical and mental health conditions

How to make a referral to TAPMI

  1. Print off and complete the TAPMI referral form and fax to TAPMI Central Intake at 416-323-2666. If you have questions about your patient’s referral, please call 416-323-6269.
  2. Your patient’s referral will be assessed by a TAPMI Central Intake team member at Women’s College hospital and sent to the appropriate clinic with the next available appointment.
  3. Your patient will be contacted directly with an appointment by the clinic and then you will be notified via fax once an appointment has been booked.

Your patient will be contacted directly by the clinic when their first appointment is booked. We will also notify you via fax.

eConsult

eConsult enables requesting clinicians, family physicians and nurse practitioners to engage in a secure, electronic dialogue with specialists to manage patient care, without the need for a face-to-face visit. eConsults may avoid the need to refer a patient to a specialist for diagnosis and treatment. eConsult is available through Ontario Telemedicine Network.

Through this platform you can email a TAPMI pain specialist and receive one time answers to your questions.

TAPMI Mobile Discharge Clinic

Need assistance with 3-6 complex chronic pain patients? The TAPMI Mobile clinic consists of:

  • Interprofessional case consulations on site in your practice.Members of the TAPMI team will come to your practice to provide Interprofessional case consultations  with you and your most complex patients. The TAPMI mobile team may include a pain physician, pharmacist, social worker and/or an occupational therapist.
  • During the consultations TAPMI team will work with you and your patient(s) to develop a plan to manage their chronic pain.

Continuing Medical Education (MainPRO-C eligible)

You will receive a 1 hour education session on chronic pain management

Criteria for participation:

  • Must be a primary care provider
  • 3-6 complex chronic pain patients that need additional chronic pain support
  • Clinic must be located within the Greater Toronto Area
